Please note that the "Name of member" field on this form is required so that we may note your payment in Lodge records. Be sure to include any suffixes such as Sr., Jr., III, etc.

Per Capita

For those brethren who have an Endowed Membership, continue to show your support to your lodge, by offering to pay your per capita donation of $30 (including fees) each year. This helps out the lodge financially.

Endowed Membership

Pay a one-time fee of $1,200 and never pay dues again. After your death, the Lodge still benefits from your endowed membership.
Please contact the Lodge Secretary if you would like to take out an endowment with the Lodge. At this time, the Lodge is only accepting checks as a form of payment for an Endowed Membership.
